Filing an appeal

• Your eligibility to enroll in a qualified health plan through Get Covered Illinois
• Your eligibility for Advanced Premium Tax Credits (APTC) or cost-sharing reductions (CSR)
  • The amount of Advanced Premium Tax Credits (APTC) 
  • The level of cost-sharing reductions (CSR)
• Your eligibility for a special enrollment period
• The effective date of your coverage or Advanced Premium Tax Credits
• Failure by Get Covered Illinois to provide you timely notice of an eligibility determination 
• Denial of a request to vacate a dismissal issued by the Get Covered Illinois appeals program 
• Termination of coverage due to a marketplace eligibility determination

Please note that only final eligibility determinations from Get Covered Illinois are appealable.

You have 90 days from the date of your notice of determination the letter to submit an appeal request for yourself or anyone in your household who applied for health insurance or financial help.

You can file an appeal by filling out an Appeals Request Form and submitting it in any one of the following ways:

Online

1. Log in to your Get Covered Illinois account.
2. Navigate to the My Tickets section. 
3. Click on Submit New Ticket. 
4. Select your request type and request subtype: 
   1. Request Type: Issue 
   2. Request Subtype: Complaint or Question 
5. In the subject line, enter: Marketplace Appeal 
6. In the description box, briefly explain the reason for your appeal. 
7. Submit the ticket. Once submitted, you’ll receive a Ticket ID. 
8. Attach your Appeals Request Form and any supporting documentation to the ticket using the Add Attachment feature.
